A Stage 4 Breast Cancer Nonprofit
 
 

The following guidelines should be reviewed by the family or the family member in charge of the welfare of the children if the mother succumbed to her Stage 4 Breast Cancer after having participated in our LESLIE'S WEEK RETREAT4STAGE4. It is the responsibility of the family to apply for Educational Assistance when their child graduates from high school and gains acceptance to an institution of higher learning up to a 4-year Bachelor's Degree; College, University, or Trade School.

  • Our LESLIE’S WEEK children who are 12 years old or less at the time of their family’s selection for the LESLIE’S WEEK RETREAT4STAGE4, having attended the RETREAT with their mother or with their father if their mother was deceased before the dates of the RETREAT, are eligible applicants. They must supply the following documentation:

    • Candidate Authorization Form which was submitted by the mother preceding the RETREAT4STAGE4. If unavailable contact LESLIE’S WEEK who has a copy in their files.

    • Copy of High School Diploma or Equivalent.

    • Copy of Letter of Acceptance from an Accredited Educational Institution

    • Two Letters of recommendation for this honor in their Mother’s name.

  • The application must be submitted on or before June 30th of the year of graduation.

    OPTION 2: Apply Through Mail
    Print and submit the completed application and required documentation and send via USPS mail to
    LESLIE’S WEEK, 17329 Corsini Drive, Fort Myers, FL 33913.

  • The LESLIE'S WEEK Educational Assistance Fund Chairwoman and her committee review the applicants and submit a response letter for Educational Assistance by August 1st.

    • Award amounts are based on the LESLIE’S WEEK Educational Assistance Criteria listed above. Applicants must have submitted all required documentation with their application to qualify.

    • Educational Assistance awards are based on LESLIE’S WEEK’S financial ability to fund the awards. The present base amount is $1,000.

    • Funds are made payable to the Institution and mailed directly to the applicant in honor of their mother

    • Awarded funds may only be applied toward tuition or program-approved books/supplies.

    • Awarded students may apply for LESLIE’S WEEK Educational Assistance awards annually while maintaining a B+ GPA (confirmed by transcripts). Availability depends upon LESLIE’S WEEK’s financial ability to fund the awards.

  • All applicants will be sent confirmation of their application status via email by August 1st.
